Position Summary
The Tenancy Support Case Manager works as a participant-centered advocate to promote housing stability and long-term housing success for households participating in the HOME Tenant-Based Rental Assistance (TBRA) Program. The Tenancy Support Case Manager assists participants in identifying and securing appropriate housing while providing comprehensive tenancy support services, including individualized case management, housing stabilization planning, landlord coordination, and connections to community resources.
Working collaboratively with the City of Fairfield, landlords, property managers, and community partners, the Tenancy Support Case Manager proactively prevents housing crises, strengthens tenant-landlord relationships, and supports participants in achieving greater independence. Throughout the two-year program, the Tenancy Support Case Manager provides ongoing supportive services designed to maximize housing retention and prepare households for a successful transition beyond HOME rental assistance.
Essential Duties & Responsibilities:Providing comprehensive tenancy support services to assist program participants in obtaining, maintaining, and successfully transitioning through housing during the two-year HOME Tenant-Based Rental Assistance program.Developing individualized Housing Stability Plans in collaboration with participants that identify goals, barriers, supportive service needs, and strategies to promote long-term housing stability.Coordinating with the City of Fairfield, property owners, landlords, property managers, and community partners to support successful housing placements and ongoing tenancy.Assisting participants with locating and securing appropriate housing that meets program eligibility requirements and aligns with their individual needs and preferences.Providing education and coaching on tenant rights and responsibilities, lease compliance, budgeting, financial literacy, housekeeping, communication with landlords, and independent living skills.Conducting regular home visits and participant meetings to monitor housing stability, assess ongoing service needs, and provide supportive case management throughout program participation.Assisting participants in accessing mainstream benefits, healthcare, behavioral health services, employment services, education, childcare, transportation, and other community resources that support long-term housing stability.Working collaboratively with landlords and property managers to proactively address tenancy concerns, resolve conflicts, and implement strategies that promote successful housing retention and prevent eviction.Providing crisis intervention, problem-solving, and linkage to community resources to assist participants in overcoming barriers that may jeopardize housing stability.Monitoring participant progress toward individualized housing stability goals and adjusting service plans as needed to address changing circumstances.Maintaining accurate, timely, and complete participant documentation, case notes, and program records in accordance with HOME Program requirements, City of Fairfield policies, and agency standards.Coordinating with the City of Fairfield regarding participant eligibility, program compliance, housing status, and successful program completion.Tracking participant outcomes, housing retention, service utilization, and program performance measures to support reporting requirements and continuous quality improvement.Maintaining knowledge of community resources, affordable housing opportunities, landlord partnerships, and changes to local housing and supportive service systems that may impact program participants.Participating in multidisciplinary case conferences, community meetings, and program planning efforts to strengthen service coordination and improve participant outcomes.Perform other related duties and assignments as required or requested.Qualifications & Skills
