Position Highlights

As a Family Support Care Navigator you will perform essential tasks related to UCP’s DDS Agency with Choice, CBDS, CIES, IFFS, and DESE contracts in compliance with state regulations and guidelines. This includes budgeting, navigation, Plans of Care, member schedules, life skills development, and Charting the Life Course. The goal of your role will be to provide support, information, resources, and navigation of care systems to families and members with disabilities.

Qualifications:
  • Bachelor’s degree and at least two years of relevant experience working in the community with elders, disabled adults or other vulnerable populations.
  • Ability to work flexible hours
  • Analyzing data, effective listening, facilitating meetings, monitoring activities, planning and problem solving and a working knowledge of computers and basic computer programs.
  • Knowledge of age-appropriate activities and behaviors, technical writing, defining issues and drawing conclusions, treatment modalities and assessments.
  • Knowledge of community social services, education and health service agencies
  • Valid driver’s license, availability of private vehicle, up-to‑date car insurance
  • Participation in a rotating on‑call schedule for the department’s emergency on‑call cell phone.
  • Proficiency with Microsoft Office/Office 365 and word processing skills
  • Comfortable with remote application such as Zoom for team or other meetings
