What You’ll Do

As a Case Manager at the Crisis Intervention Center
, you’ll play a vital role in guiding individuals through some of the most challenging moments of their lives. You’ll work closely with a multidisciplinary team to ensure clients receive the care, resources, and support they need both during their stay and after discharge. Here’s what your day‑to‑day will look like:

  • Coordinate admissions, transfers, and discharge planning in collaboration with the Clinical Director and care team.
  • Serve as a key liaison between clients, families, legal reps, and community service providers.
  • Develop and implement person‑centered treatment and discharge plans focused on recovery, stability, and long‑term success.
  • Identify appropriate services or placements following discharge and ensure timely, effective referrals (housing, outpatient care, substance use treatment, etc.).
  • Advocate for clients’ rights and ensure their voice is heard throughout their care journey.
  • Maintain up‑to‑date and accurate documentation of all case coordination efforts in compliance with HIPAA and KDADS regulations.
  • Work to remove common barriers to discharge such as lack of housing, transportation, or insurance.
  • Support the development of client‑centered tools like WRAP (Wellness Recovery Action Plans).
  • Complete all required training in trauma‑informed care, de‑escalation, crisis response, and more.

Qualifications
  • Experience counts! We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college.
  • Must be at least 21 years old.
  • Experience in mental health, crisis intervention, or substance abuse treatment preferred.
  • Ability to handle high‑pressure situations and respond to emergencies.
  • Must pass KBI and KDADS background checks.
  • Must pass drug test.
  • Maintain valid driver’s license and insurable driving record (no substance‑related incidents within 5 years).
  • Must meet all health screenings, TB testing, and physical clearance requirements.
