Job SummaryThe Sleep Care Advanced Practice Provider works a daytime schedule in an ambulatory setting managing initial patient consultations and ongoing follow‑up visits to provide quality health care for Sleep Center patients. The Sleep Disorders advanced practitioner engages in independent decision‑making regarding the health care of individuals, families, and groups across the health care continuum, with a primary focus on Sleep Medicine.
The APRN collaborates with the Sleep Center’s physicians, clinical and administrative staff, the patients and their families to facilitate and improve patient care, and provides patient education to enhance long‑term health. At Hartford Health Care Sleep Center, newly graduated and experienced practitioners receive support and mentorship through on‑the‑job, classroom, and procedural training.

- Graduate of an accredited Nurse Practitioner master’s program
- Licensed or eligible to be licensed as an Advanced Practice Registered Nurse (APRN) in the state of Connecticut
- Certification, or eligible to be certified, as National Board Certified Ambulatory Advanced Practice Registered Nurse
- Sleep medicine experience preferred
- Bilingual preferred but not required
