Job Summary

We are seeking a compassionate and skilled Physician Assistant or Nurse Practitioner to join our team. The ideal candidate will have expertise in wound care but is not required; on the job training will be provided as necessary. As a key member of our wound care team, you will play a vital role in assessing, treating, and managing complex wounds, while collaborating closely with physicians, nurses, and other healthcare professionals.

Location

& Schedule
  • Part Time, 16-20 hours per week
  • Primarily office-based, including locations:
    Fairfield, Trumbull, Shelton, and Orange with possible rounding at local acute rehab facilities.
Essential Responsibilities
  • Conduct comprehensive wound assessments and develop individualized treatment plans based on evidence‑based guidelines and best practices.
  • Provide wound care procedures/treatments autonomously including debridement, I&D, wound culture, etc.
  • Utilize advanced wound care modalities, such as negative pressure wound therapy, bioengineered skin substitutes, to optimize wound healing outcomes.
  • Educate patients and caregivers on proper techniques, prevention strategies, and the importance of adherence to treatment plans.
  • Utilize electronic health record systems for documentation.
  • Collaborate with multidisciplinary teams to ensure optimal patient outcomes.
  • Maintain accurate and thorough documentation of patient assessments, interventions, and outcomes in accordance with regulatory requirements and organizational policies.
  • Demonstrates organization, flexibility, and cooperation in performing job responsibilities.
  • Prescribe medications, including narcotics, controlled substances or class scheduled drugs with Supervising Physician’s documentation in the patient medical record.
  • Perform other duties as assigned by their supervisor.
Education & Experience Requirements
  • Valid Nurse Practitioner or PA-C license in state of Connecticut
  • Valid Controlled substance and DEA license
  • 2+ years of wound care experience preferred
