Salary: $115,000 – $175,000 per year (based on experience + achievable monthly bonuses)

- Perform comprehensive wound assessments and develop individualized treatment plans
- Provide advanced wound care services, including debridement, dressing selection, and infection management
- Monitor healing progress and adjust treatment as needed
- Collaborate with facility staff to ensure continuity of care and prevention strategies
- Educate patients, families, and staff on wound management and prevention
- Complete documentation within 24 hours
- Ensure compliance with all state, facility, and clinical guidelines
- Active Nurse Practitioner (NP) license in the state of hire
- 2 years of experience preferred but not required
- Preferred experience in:
Skilled Nursing Facilities (SNFs), Wound care and/or wound certification (WOCN, WCC, etc.), Long-term care, acute care, or rehabilitation settings - Proficiency in Electronic Health Records (EHR)
- Strong communication skills and a patient-centered mindset
Schedule:
Monday–Friday, 8 AM – 5 PM, Fairfield, CT
