We are seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our growing clinical team serving patients within skilled nursing care facilities in Fairfield, CT!!

Impact Health PC is a leading advanced practitioner group in skilled nursing facilities across the Northeast, that is dedicated to prioritizing patient care. We specialize in Medical, Psychiatric, Wound Care and Palliative Care.

Compensation and Benefits:

Earning potential:
From $110,000 up to $200,000 (salary based on experience + easily achieved monthly bonuses)
Flexible

Schedule:

Monday – Friday. No on-call, weekends, or holidays.

Paid training and onboarding

Ongoing education and clinical support, including physician-level consult access

Autonomy with Support to deliver independent care with access to physician-level consults.

Key Responsibilities:

Assess, develop and communicate comprehensive treatment plans using evidence based best practices

Documentation withing 24-48 hours along with placing relevant orders for diagnostics and medications

Monitor and manage acute and chronic conditions

Compliance with facility, state and federal regulations/protocols.

Support quality initiatives and process improvement plans.

Collaborate with interdisciplinary team, nurse managers, physicians and specialists

Educate and consul patients and families regarding course of treatment

Qualifications:

Active license as a Nurse Practitioner (NP) or Physician Assistant (PA)
Minimum 2 years of post-certification experience

Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred

Proficient with EHRs and clinical technology

Strong communicator with a collaborative mindset#MP123
