Faith Home Healthcare, a division of Faith Health Group, is seeking an experienced and compassionate Registered Nurse (RN) Manager to oversee and manage our Private Duty Nursing (PDN) home care program. The Registered Nurse (RN) Manager will be responsible for clinical oversight, supervision of nursing staff, clinical education, compliance with state and payer regulations, coordination of patient care, and ensuring the delivery of high-quality, patient-centered nursing services in the home setting.
This Registered Nurse (RN) Nurse Manager is ideal for a strong clinical leader who is organized, detail-oriented, and passionate about delivering excellent care to medically complex pediatric and adult patients.
OverviewKey Responsibilities Clinical Oversight & Patient Care
- Oversee all Private Duty Nursing (PDN) home care cases, including pediatric and adult clients
- Conduct initial assessments, reassessments, and supervisory visits as required
- Develop, review, and update individualized Plans of Care (POC)
- Ensure all nursing services comply with physician orders and care plans
- Provide clinical guidance, education and support to field nurses
- Supervise RNs and LPNs providing PDN home care services
- Conduct competency evaluations, performance reviews, and clinical supervision
- Participate in hiring, orientation, and ongoing training of nursing staff
- Address clinical concerns, incidents, and corrective actions as needed
- Ensure compliance with NJ Department of Health regulations, Medicaid, insurance requirements and accrediting body standards
- Review and approve nursing documentation, notes, and timesheets
- Ensure EVV compliance and accurate clinical records
- Participate in audits, surveys, and corrective action plans
- Collaborate with physicians, case managers, families, and internal departments
- Communicate effectively with caregivers and families regarding care needs
- Support intake, case openings, and staffing coordination
- Monitor quality of care and patient outcomes
- Investigate incidents, variances, and complaints
- Maintain high standards of patient safety and satisfaction
- Active Registered Nurse (RN) license – State of New Jersey (required)
- Minimum 2–3 years of nursing experience, preferably in:
- Private Duty Nursing (PDN)
- Home Care
- Pediatric and adult medically complex care
- Prior supervisory or management experience preferred
- Strong knowledge of:
- Medicaid PDN guidelines
- Plan of Care development
- Clinical documentation standards
- Excellent communication and leadership skills
- Strong organizational and time-management abilities
- Proficient with EMR/EVV systems
- Valid driver’s license and reliable transportation
- Pediatric PDN experience
- Ventilator, tracheostomy, and G-tube care knowledge
- Experience working with Medicaid and managed care organizations
