Job SummaryProvides safe, competent care for persons with wound, skin and ostomy needs. Assesses, plans, implements and evaluates treatment of wound, skin and ostomy patients in collaboration with the healthcare team. Facilitates education and training, planning, organization, and implementation within the hospital. Acts as a resource for hospital staff regarding wound/skin/ostomy care. Participates in performance improvement processes to ensure competent nursing care related to wound, skin, and ostomy care.
EssentialFunctions
- Conduct a thorough, individualized assessment of the person with or at risk of wound/skin/ostomy issues.
- Collaborate with the patient, healthcare team, and/or referring physician to identify treatment plans.
- Analyze assessment data accurately and in a timely manner, and establish or modify a treatment plan in collaboration with the medical team.
- Develop, maintain or modify a plan of care and communicate it to appropriate members of the health care team.
- Implement and monitor skin preservation strategies, delegating nursing duties when appropriate.
- Evaluate the quality and outcomes of current interventions, documenting and communicating responses to the healthcare team; reassess the patient response and revise treatments as needed.
- Educate the patient, families, and other support persons on ulcer prevention, treatment, and ostomy care.
- Provide ongoing education to hospital staff related to wound and skin care.
- Assess employee competency on assigned units and develop competency assessment plans related to wounds, skin, and ostomy care.
- Participate in the development, implementation, and evaluation of regional education programs and orientations related to wound, skin and ostomy care.
- Identify patient education needs and facilitate the development and availability of patient educational material.
- Provide quarterly pressure ulcer and skin assessments for the hospital.
- Conduct learning needs assessments to determine educational needs annually and as needed.
- Actively participate in performance improvement.
- Assist staff in meeting professional and departmental standards of care and organizational policies, utilizing feedback and coaching to enhance quality of care.
- Participate in the development, implementation, and evaluation of policies and procedures.
- Participate in local and regional wound care committees.
- Identify and participate in the performance improvement process.
- Associate’s Degree (2 years) with ability to obtain a BSN within 3 years of hire.
- Preferred:
Bachelor’s Degree (4 years).
- Registered Nurse (RN) licensed in Ohio.
- WOCN certification (must be active).
- At least 2 years of RN experience.
- Must be willing to obtain WOCN certification within 2 years of employment.
- Strongly prefer 3 years of nursing experience in an acute care setting.
