Job Summary

Human Services of Faribault & Martin Counties seeks a full‑time (39 hours a week) Case Manager (CPS) - Family Preservation Worker position. This position is located in our Faribault County Human Services Center (Blue Earth, MN).

We are seeking a dedicated and compassionate candidate to engage families involved in child protection, ensuring the safety, permanency, and well‑being of children. The ideal candidate will develop and monitor case plans, collaborate with families, and complete necessary documentation to meet state department/agency regulations. Your duties will include meeting with families, providing support, conducting assessments, and processing thorough documentation. You will also participate in meetings and training, educate the community about neglect and abuse, maintain professional relationships with coworkers, and ensure compliance with relevant laws and policies.

Effective communication, respect for clients, and adherence to privacy rules are key aspects of this role.

The HSFMC comprehensive benefit package includes health insurance (including option for high‑deductible health plan) with employer sharing in cost, employer‑paid $20,000 term life insurance, earned sick and safe time, generous paid leave policy, and 11 paid holidays each year. • Interviews clients at intake.
• Carries a caseload where a need for continuing social case work predominates.
• Provides counseling to families and individuals.
• Conducts complex case evaluation for the purpose of assessing problems and determining appropriate types and methods of treatment.
• Prepares intensive long or short‑term treatment plans, requiring a fund of casework knowledge.
• Identifies client situations requiring intensified service and brings to the attention of supervisor for help or referral.
• Provides adult and child protective services.
• Provides vocation/employment services.
• Provides adult and children's mental health services and services to persons with disabilities.
• Provides or assists in providing services to other cases involving difficult or complex social and financial problems.
• Interprets programs to clients, refers clients to appropriate community services.
• Interprets social and emotional factors to others involved in treatment of clients.
• Prepares social histories with emphasis on psychosocial factors, adoption, or foster home placements.
• Supervises foster home placements.
