Registered Nurse (RN) – Home Health

Schedule: Monday–Friday, 8:00 AM–5:00 PM
Hours: 40 hours guaranteed
On-Call: Rotating every 3rd weekend
License: MD RN or Compact RN
Productivity: 30 points/week
Mileage: $0.43/mile
County: Washington County

Seeking an experienced Home Health RN to provide skilled nursing care, assess patient needs, manage care plans, and coordinate services with physicians and other healthcare professionals. The RN will support health maintenance, disease prevention, and positive patient outcomes.

• Assess patients and develop, implement, and evaluate individualized care plans.
• Provide skilled nursing care and monitor changes in patient condition.
• Coordinate care with physicians and other healthcare providers.
• Manage medications, physician orders, appointments, and follow-up care.
• Complete required clinical documentation and maintain accurate patient records.
• Coordinate care during hospitalizations and discharge.
• Supervise LPN/LVN staff and assist with training as needed.
• Maintain compliance with clinical, health, and safety standards.

• Active MD or Compact RN License
• HCHB and OASIS experience required
• PICC line experience required
• Wound VAC experience preferred
• Strong clinical assessment, documentation, and communication skills
• Ability to meet 30 productivity points/week
• Ability to participate in rotating weekend on-call coverage
