Position: Weekend On Call RN/LPN-Scheduled 24 Paid for 30-FT Benefits

Position:
On‑Call Nurse – Hospice Services

What You Will Do
  • Assess physical, psychosocial, and spiritual needs of the patient and family (RN only).
  • Gather data related to patient and family’s physical, psychosocial and spiritual needs (LPN).
  • Provide intermittent skilled nursing services to the patient under the patient’s physician’s direction.
  • Monitor and evaluate pain and symptoms; provide information and assistance to the patient and family to manage those problems.
  • Serve as a liaison to the patient’s physician(s) when needed; provide information to the physician based on knowledge relating to the patient’s needs.
  • Provide emotional support to the patient and family.
  • Refer patients and/or family members to other services when appropriate.
  • Provide instruction (healthcare teaching) to the patient and family as related to care in a residence.
  • Make visit to patient’s residence at the time of death.
  • Maintain, promptly complete, and return all documentation as required.
  • Communicate and act as liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity.
  • Relay pertinent information to appropriate team members.
  • Participate in meetings as initiated by team leader.
  • Participate in mandatory or elective HWR University events, trainings, or in‑services as necessary.
  • RN’s shall participate in triage coverage as needed.
  • Perform other duties as assigned.
Success Capabilities
  • High School Diploma or equivalent required.
  • Graduate of an accredited school of nursing, with a Bachelors degree preferred.
  • Licensed RN/LPN in state of Ohio.
  • Experience preferred in pain and symptom management, oncology, or mental health.
  • Ability to communicate effectively, orally and in writing.
  • Ability to work independently and collaboratively with a team.
  • Must have demonstrated knowledge and skills to provide appropriate care to all age groups.
  • Ability to handle stressful and constantly changing conditions; must be able to make good professional decisions quickly despite stressful atmosphere.
  • Ability to access patients and families in any residential and/or public setting in any weather condition (i.e., climbing stairs or snow‑filled walkways, etc.).
  • Proficiency in Microsoft Office suite of products, particularly proficient in Word and Excel, use of shared team files, a donor database, grant portals and virtual meeting technology.
  • Must have and maintain a valid driver’s license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR’s insurer.
Physical Requirements
  • Ability to stand and walk for extensive periods of time.
  • Manual and finger dexterity and eye‑hand coordination to provide nursing care and operate equipment.
  • Heavy work requiring the exertion of up to 75 lbs. of force occasionally and 50 lbs. frequently, using appropriate safe transfer techniques and body mechanics when working with patients.
  • Ability to stoop, squat, or bend frequently.
  • Corrected vision and hearing within normal range to observe and communicate with patients and professional staff.
  • Possibility of exposure to environmental elements, internal and external, such as disease, blood borne pathogens, chemicals and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations.

Conditions of Employment
  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation.
  • Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed.

  • Ability to provide proof of eligibility to work in the United States.
