We offer complete post-acute and rehab medical services to both nursing facilities and skilled nurse facilities. Our physicians provide guidance and leadership on the use of medicine in your facility. We give supportive care to patients in the final phase of a terminal illness and focus on comfort and quality of life rather than cure.

About the role:

To take care of patients in the nursing home setting. Long-term care and skilled patients. Working with an APP, diagnosing, treating, and coordinating the care of these patients. We are looking for a highly motivated and caring provider who's passionate about delivering the highest level of care possible for their patients. The coverage is mainly Monday
- Friday position.

Required Qualifications:

• 1+ years of professional experience
• Experience using electronic health record (EHR) software
• Ability to learn/stay up to date on relevant practices
• Critical thinking is needed when issues or concerns arise with patients and families of patients
• Detail-oriented, organized, and with a high degree of accuracy

Preferred qualifications:

• Post acute care/ skilled nursing facility experience
