Summary
Under the direction of the Medical Records/HIM Director, the HIM Processing Associate is responsible for collecting, compiling, processing, storing, and maintaining medical records of hospital and clinic patients, ensuring compliance with medical, administrative, and legal requirements.
Responsibilities- Collect all current and previous medical records of discharged patients from all service areas within 24 hours of discharge.
- Print and reconcile the daily discharge list in alphabetical order.
- Follow up on expired patients or death certificates and complete the missing record tracking sheet as needed.
- Attach loose documents, remove duplicates or blanks, and review documents for advance directives and consents.
- Verify patient information in Meditech, assemble records in order by service delivery, and review for completeness, authentication, and presence of all test results and documentation.
- Validate discharge transfer orders (IP & DSU) and enter deficiencies in Meditech.
- Create temporary folders for incomplete processes and maintain incomplete records in TDO order by physician.
- Complete secondary analysis and check PDOCs/Dictation reviews, updating deficiencies as incomplete charts are processed.
- Scan all records into Meditech and perform indexing/quality assurance reviews.
- Handle telephone, fax, walk‑in, death certificate, healthcare proxy inquiries, and other related tasks.
- Answer phones, process release of information requests during off hours, and review admission notices for HPC/DNR.
- Perform chart pulls or copies as required, re‑file records, and log order charts for off‑site storage.
- Log and file sane kits, retrieve HCP from admitting precopy and index if necessary.
- Shutdown shelves, purge and shred paper records, and maintain a courteous demeanor with patients, visitors, and staff.
- Follow Steward and hospital policies, complete mandatory education in Steward University, and refer potential medical/legal issues to the Director and Risk Management.
- Knowledge of medical terminology and healthcare facilities.
- Previous office experience, preferably in a hospital setting.
- Cooperative, courteous, respectful, and compassionate with a strong customer focus.
- Effective communication skills with diverse cultures, values, and ages.
- Patience, timely response to requests, and skillful handling of interruptions.
- Professional appearance and demeanor.
- Ability to prevent and resolve customer concerns, maintain confidentiality, and accept responsibility for actions.
- Commitment to ongoing development and participation in staff meetings, orientation, and mentoring.
- Analytical and decision‑making skills with minimal supervision.
- Understanding of hospital and departmental policies, including safety issues.
- Awareness of hospital programs and services.
High School Diploma or equivalent.
Physical Requirements75% or more:

Stand, walk, use hands, climb or balance. Lift up to 25 lbs. Vision: close vision, distance vision, peripheral vision, and ability to adjust focus.
Hourly rate: $17.55–$25.57.
LocationSaint Annes Hospital – 795 Middle Street, Fall River, Massachusetts 02721.
