Open Sky's Day Habilitation programs serves adults between the ages of 23-85 with a primary diagnosis of intellectual disabilities, though some individuals may also have mental health diagnoses and/or physical challenges. Most participants require supportive structures and staff assistance to maximize their involvement in activities within the program as well as within the local community. Our staff educate through the creation and implementation of individualized service plans, daily curriculum/activities, and have a heavy focus on community integration.
As a Day Hab Support Staff, you will provide support to the individuals served in a direct care role by ensuring a supportive and structured environment. They also provide guidance, advocacy, and life skills education for individuals served within the Day Program.
Other Key Responsibilites- Implement all service and behavior plans with support from the interdisciplinary team
- Review and complete all required documentation.
- Ensure the health, safety and well-being of all individuals.
- Facilitate the development of valued community roles and personal relationships
- Participate in team meetings, supervisions, and other assigned meetings.
High School Diploma, GED or equivalent, required.
Valid Drivers License and acceptable driving record, required.
Drug testingrequired.
Reliability, willingness to learn, and being open to new opportunities.
