Case Management Supervisor – Outpatient Behavioral Health
We’re seeking a Case Management Supervisor to lead and support our outpatient case management team. This leadership role is ideal for someone who is passionate about improving access to care, streamlining operations, and enhancing the patient experience across behavioral health services.
As the Case Management Supervisor, you will oversee care coordination, patient engagement, and workflow processes that ensure continuity of care for individuals receiving therapy and psychiatric services. This is a non‑clinical supervisory role focused on operational excellence, team leadership, and service coordination.
Leadership & Team Supervision- Supervise outpatient case managers, providing guidance, training, and performance management
- Oversee daily workflows, task assignment, and team coverage
- Promote consistency and efficiency across case management operations
- Lead administrative workflows related to patient access, care transitions, and service coordination
- Collaborate with admissions, front desk, billing, and clinical teams to optimize patient flow
- Identify process gaps and implement workflow improvements
- Track and analyze key operational metrics such as:
- No-show rates
- Patient engagement and reactivation
- Transitions of care
- Discharges
- Documentation timeliness
- Unbilled services
- Report trends and recommend improvements to leadership
- Provide backup support for care coordination as needed
- Assist with complex referrals, transitions of care, and service navigation
- Serve as a resource for staff on workflows, community resources, and patient engagement strategies
- Facilitate communication between patients, providers, pharmacies, hospitals, and community agencies
- Monitor documentation timeliness and service capture
- Identify workflow gaps affecting billing and operations
- Conduct or support chart audits
- Meaningful work supporting access to behavioral health care

- Bachelor’s degree in healthcare, behavioral health, business, or related field
- 3-5 years in healthcare, behavioral health, case management, or care coordination
- Strong understanding of outpatient behavioral health workflows
- Experience with care coordination, patient engagement, and transitions of care
- Familiarity with documentation, billing processes, and service tracking
- Knowledge of HIPAA and 42 CFR Part 2
- Strong leadership and team management abilities
- Data tracking and reporting (Excel preferred)
- Excellent communication and problem‑solving skills
- Highly organized with strong time management
- Ability to work collaboratively across multidisciplinary teams
