- Preferred Qualifications:
- Experience in SNF or rehabilitation environments
- Strong documentation and communication skills
- Additional Information:
Responsibilities include screening, evaluating, and treating patients with physical functioning disorders, implementing evidence‑based interventions, and documenting all clinical activities in compliance with regulatory standards. Candidates must supervise support staff, participate in team meetings, and support the facility’s quality initiatives.
