Starting hourly rate $20 per hour with potential to exceed state average. Extra pay for night shifts (extra $3.50/hr weekdays 7pm‑7am Monday‑Friday) and weekend shifts (extra $4.00/hr Friday 7pm‑Monday 7am).
Mileage Reimbursement
Respectful and desirable work setting:
Predictable hours, autonomy, low‑volume, member‑focused care.
Provide personal care, light housekeeping and assistance with ADLs (Activities of Daily Living) as outlined in each participant’s care plan in participant homes and at the PACE center; exercise independent judgment; report changes; participate in infection control and cleaning; follow guidelines.
Responsibilities- Under RN supervision, assist with ADL needs of participants at the PACE center and in participants’ homes.
- Contribute to the development of a care plan for participants through interaction with the Interdisciplinary Team.
- Collaborate with team members to assure appropriateness and continuity of care.
- Carry out non‑skilled treatments including vital signs, transfers, toileting, bathing, dressing.
- Assist with recreational activities and group programs, helping plan programs, motivating participants, evaluating effectiveness.
- Assist registered therapists and therapy assistants with treatments and activities ordered by the therapist.
- Maintain a clean and safe environment in the PACE center and participants’ homes.
- Assist with meal and snack preparation, serving, feeding, and clean‑up.
- Use safe techniques in all interactions with participants.
- Provide accurate and timely documentation in EMR and other systems as required.
- Participate in daily cleaning and disinfecting of the PACE center per CDC guidelines.
- Participate in distribution of work for health aides to ensure timely completion of site tasks.
- Participate in regular team meetings for health aides at the site.
- Perform duties in accordance with FH and Summit Elder Care policies.
Education: Completion of an approved Home Health Aide or CNA training program is desirable, but equivalent work experience will be accepted.
Certification: Certification as a Home Health Aide or Nursing Assistant is desired.
Reliable transportation required.
Experience: One year working with frail or older adults, preferably in a PACE program. If less than one year, must complete Summit Eldercare training prior to independent work. Must complete a standardized set of Summit Eldercare competencies annually.
