New Per Visits Rates!

VNA Care is hiring Per Visit PTs;
Home Health experience required.

Work any combination of days and hours, work full days, half days or just for a visit. Visits are scheduled between 8a-4:30p Mon-Fri and between 8a-6:30p Sat-Sun.

Territory:
Concord / Acton Team, Physical Therapy visits are scheduled in a tight geographic area for each day.

Position Summary:

Provides home health care to patients and families that focuses on preventing disease, promoting, maintaining or restoring health and minimizing the effects of illness or disability.

Experience Preferred

• Minimum 1 year in acute care setting or comparable work experience.
• Home Health experience required.
