A great SNF / LTC Facility in Shreveport, Louisiana is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team on a contract assignment. The Physical Therapist is responsible for the assessment of referred patients who require rehabilitation services and will provide direct patient care.

• Assess patients’ medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals based on the patient’s capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for patients.
• Document and record patient condition.
• Educate patients and families on appropriate physical therapy methods.

• Graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in the process of obtaining one.
• Current CPR certification.
• Develop a plan of care for each patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet therapy goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
