Signify Health, part of CVS Health®, is seeking a Full-time Clinician (NP or PA) to provide in‑home health evaluations at members' residences. Each visit lasts about 45 minutes and includes health questions, medication/history review, and a physical evaluation.
We travel locally within licensed states and may conduct virtual visits. Clinicians use an iPad workflow and coordinate care to bridge gaps in care for health plan members in Massachusetts and surrounding areas.
