Overview

Full-Time Physical Therapist - Home Health in Massachusetts to provide care to adult and geriatric patients in their homes in Sturbridge, Southbridge, Charlton, Dudley and surrounding towns. Prior home care experience is preferred but not required. You may be eligible for a $10,000 Sign-On Bonus.

BAYADA Home Health Care is seeking an experienced Physical Therapist to be an integral member of a multi-disciplinary health care team that delivers skilled nursing and rehabilitative care to clients, helping them maintain independence.

• Make visits to clients in their homes.
• Perform diagnostic tests and measurements (mobility and range of joints, transfer status, stability, ambulation patterns, strength and endurance, balance and safety assessments).
• Develop and implement individualized care plans, including manual therapy, gait training, balance training and other interventions.
• Continually assess and revise the Physical Therapy care plan and participate with nursing in the multidisciplinary care plan as appropriate.
• Educate and instruct clients, family members or other representatives in rehabilitative care to promote health, safety and independent living.
• Accurately document observations, interventions and evaluations using the designated documentation system.

• A current license as a Physical Therapist in Massachusetts
• A minimum of one year of recent clinical experience as a licensed Physical Therapist
