Direct Care Support-Shared Living
- Part Time

Pay $19.75 / hour

Schedule:

Intermittent Morning hours (20 hours)

The Direct Care Professional (Shared Living) implements individualized programs as specified in Individual Service Plans, provides for individuals' health and welfare, and ensures the safety of individuals.

Essential Functions:

  • Work as part of a team of family members and other professionals who are involved in the individual's care and training.
  • Learn the goals of the individual and work to help individual meet them. Follow a behavior plan if required.
  • Provide care for the individual in their home when appropriate, helping he/she to meet their goals within the home.
  • Plan for and provide support for community-based activities (as required) that are tailored to the individual's strengths and interests.
  • Understand and support the goals and objectives of the program.
  • Provide transportation for the individual when needed.
  • Document activities and report on activities and progress toward meeting goals.
  • Respond to requests for meetings and information from supervisor.
  • May be required to use employee's personal vehicle for work purposes. Perform other duties as assigned by the supervisor, division management team member, or division director including but not limited to working weekends and/or holidays. Participate in all staff meetings and required training sessions, including in-service training, as scheduled. Maintain all program and Agency required trainings.

Qualifications:

  • Attainment of high school diploma/GED preferred.
  • Skills obtained through Human Rights, Fire Safety, First Aid/CPR.
  • Knowledge of appropriate experience working with individuals with developmental disabilities.
  • Understanding of and commitment to the need to maintain confidentiality.
  • Knowledge of and respect for cultural diversity.
  • Skill in communicating orally and in writing.
  • Ability to work independently and as part of a team.
  • Ability to maintain a valid driver's license and reliable transportation.
  • Ability to work effectively with adults with a developmental disability.
  • A qualifying criminal background check, driving record and a valid driver's license are required.
