This position is for a Physical Therapist working day shifts from 8:00 AM to 4:30 PM in a Skilled Nursing Facility (SNF) environment with a guaranteed 35 hours per week. The Physical Therapist will provide rehabilitation services to residents recovering from injury, illness, surgery, or age-related functional decline. The role focuses on helping patients restore mobility, strength, balance, and overall independence in daily activities.

The therapist will manage a caseload of residents requiring individualized therapy plans and collaborate with interdisciplinary healthcare teams to support rehabilitation goals. The position requires strong patient interaction, clinical evaluation skills, and the ability to adapt treatment strategies to meet the needs of residents in a long-term or post-acute care setting.

Requirements:
Candidates must hold an active Physical Therapist license in the appropriate state and have graduated from an accredited Physical Therapy program with a Doctor of Physical Therapy or equivalent degree. Prior experience working in a Skilled Nursing Facility or rehabilitation setting is preferred. Applicants should demonstrate strong knowledge of rehabilitation techniques including gait training, therapeutic exercise, mobility restoration, and fall prevention strategies.

Strong communication skills are necessary to coordinate care with nursing staff, physicians, and rehabilitation team members. Candidates must also possess strong documentation skills and familiarity with electronic medical record systems to ensure accurate patient care records and regulatory compliance within the facility.
