Job description:
We are seeking a dedicated and compassionate Nurse Practitioner to join a well-established nursing home program in Fall River, Massachusetts. This role involves providing comprehensive care to patients residing in skilled nursing facilities (SNFs), working collaboratively with other Nurse Practitioners and each facility's Medical Director.
Key Responsibilities:
- Conduct patient rounds at local skilled nursing facilities
- Perform comprehensive assessments and develop individualized management plans
- Diagnose and manage acute and chronic medical conditions
- Coordinate care with Medical Directors and interdisciplinary teams
- Direct and oversee patient care plans to ensure quality outcomes
- Active Nurse Practitioner license in the Commonwealth of Massachusetts
- Valid Drug Enforcement Agency (DEA) certificate
- Massachusetts Controlled Substances Registration
- Strong clinical decision-making and independent practice skills
- Excellent communication and collaboration abilities
