In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Boston (Norfolk), MA
Signify Health, a part of CVS Health®, is seeking a Part‑Time Clinician—Nurse Practitioner or Physician Assistant—to conduct in‑home health evaluations in the Norfolk County area of Boston, MA.
Base Pay Range$46.00/hr – $99.00/hr (base pay per evaluation)
Anticipated Weekly Hours29.5 hours per week

Responsibilities- Conduct in‑home health evaluations lasting approximately 45 minutes, including medication and medical history review, physical examination, and, when ordered by the member’s health plan, diagnostic tests (e.g., diabetic eye exams, spirometry).
- Use an iPad and an intuitive clinical workflow for each evaluation.
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
- Build trusted relationships to make members healthier, helping to bridge care gaps.
- Be flexible to travel locally and within licensed states, and conduct virtual visits as needed.
- Do not prescribe medication, order lab tests, or alter existing treatment regimens.
Your primary workplace is the private residence of members, which may include pets, secondhand smoke, and other airborne allergens or irritants. Candidates must comfortably and safely perform duties in such environments.
Required &Preferred Qualifications (NPs & PAs)
- Active, unrestricted license(s) in the coverage area.
- Board certification (required).
- Multi‑state licenses (preferred).
- Ability to cover the Norfolk County area.
In addition to the base pay listed above, you may be eligible for reimbursement for completed Diagnostic and Preventive Services (DPS) as ordered.
