The Senior Care Options Assessment Nurse Case Manager will be covering and visiting members in Cape Cod. The Assessment Nurse Case Manager completes face‑to‑face home visits within 30 days of enrollment, conducts regulatory assessments, HRAs, PCA assessments, and submits MDS assessments. Work may be conducted in person or telephonically.
Responsibilities- Conduct home visits for onboarding and regulatory assessments.
- Complete HRAs and PCA assessments.
- Submit MDS assessments.
- Provide education on the Navi Care case management program.
- Conduct telephonic assessments when appropriate.
- Collaborate with Care Team.
- Complete LTSS evaluations and collaborate with UM.
- Perform medication reconciliation.
- Conduct functional assessments for LTSS programs.
- Participate in training and audits.
- Maintain program/policy knowledge to educate members.
- Support HEDIS, Medicare 5 Star, and other initiatives.
Education: Graduate from an accredited school of nursing (mandatory). A bachelor’s or advanced degree in nursing or a health‑care related field preferred.
License: Active, unrestricted Registered Nurse license in Massachusetts.
Certification: Certification in Case Management strongly desired.
Driving: Must possess a valid driver’s license, no disqualifiers per Driver Safety Policy, proof of minimal state required auto insurance, and reliable personal transportation.
Experience: 1+ years of clinical RN experience with complex medical, behavioral, and social comorbidities; ability to conduct assessments in‑person and via telehealth; interdisciplinary teamwork; screening of social determinants of health; strong communication and interviewing skills; problem‑solving and adaptability; knowledge or willingness to learn regulatory requirements. Preferred experience includes Home Health, OASIS/MDS, Medicare/Medicaid, and face‑to‑face member interactions. Reliable home internet required.
Pay Range DisclosureIn accordance with the Massachusetts Wage Transparency Act, the pay range for this position is $95,000 - $100,000 per year. Final compensation will depend on experience, skills, and fit with the role.
