In-Home Health - Nurse Practitioner or Physician Assistant; -Worcester, MA

Signify Health, part of CVS Health®, is seeking a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The visit lasts about 45 minutes, allowing time to connect one‑on‑one and answer health questions. It includes a medication and medical history review, a physical evaluation, and optionally diagnostic tests such as diabetic eye exams or spirometry.

• Engage with patients in their homes and provide personalized health evaluations.
• Conduct medication and medical history reviews, physical exams, and additional diagnostic tests as ordered by the patient’s health plan.
• Use an iPad and an intuitive clinical workflow for each evaluation; no prescribing, ordering lab tests or altering current treatment regimens.
• Travel locally and within licensed states as needed to conduct in‑home visits.
• Conduct virtual visits when required.
• Collaborate with Signify Health clinician colleagues and fulfill role requirements.
• Provide diagnostic and preventive services such as spirometry, diabetic eye exams, etc.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Qualified to cover primary locations such as Worcester, MA.

The typical pay range for this role is: $95,738.00 – $, based on experience, education, geography and other factors.
