Part-Time Clinician (Nurse Practitioner or Physician Assistant)

Signify Health, part of CVS Health, is seeking a Part Time Clinician to provide in‑home health evaluations. The role involves engaging with patients in their homes and delivering comprehensive, personalized health assessments.

• Conduct approximately 45‑minute in‑home visits that include medication and medical history review, physical evaluation, and, if ordered, diagnostic tests (e.g., diabetic eye exams, spirometry).
• Use an iPad and clinical workflow tools; you are not required to prescribe medicine, order lab tests, or change patients' existing treatment regimens.
• Participate in virtual visits as needed.
• Provide all Signify Health services including diagnostic and preventive services products (spirometry, DEE, bone density screening, etc.).
• Be flexible to travel locally and within licensed states; cover Worcester and surrounding area.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses preferred.

Part‑time employees may receive selected benefits, including malpractice insurance coverage, supplies and other perks, and pay per completed Diagnostic and Preventive Services (DPS). Anticipated weekly hours: 29.5.

Base pay per in‑home evaluation: $46.03 – $99.14 per hour (or base annual salary for full‑time equivalents). Pay depends on experience, education, geography, and other factors.
