In-Home Health - NP​/PA; Part Time- Boston; Bristol, Norfolk, Plymouth, Suffolk

Position Overview

In-Home Health – Nurse Practitioner or Physician Assistant (Part Time) – Part time clinicians in the Greater Boston Area (Bristol, Norfolk, Plymouth, Suffolk). Experience 29.5 hours per week, using advance clinical workflow on an iPad in members’ homes. The role includes virtual visits when required and requires travel within licensed states.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and optionally additional diagnostics such as diabetic eye exams or spirometry when ordered by the member’s plan.

• Conduct in‑home, patient‑centered health evaluations.
• Review medication and medical histories and perform physical examinations.
• Order additional diagnostic tests as appropriate.
• Use an iPad and intuitive clinical workflow for each visit.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Provide diagnostic and preventive services such as spirometry, DEE, bone density screening, and more.
• Build trusted relationships with patients and connect them to the right care.
• Collaborate with Signify Health clinicians to meet care goals.

• Active, unrestricted license(s) in the coverage area(s) for NP or PA.
• Board certification (required).
• Multi‑state license(s) (preferred).
• Ability to cover Bristol, Norfolk, Plymouth, and Suffolk counties.

Typical hourly rate: $46.03 – $99.14. The final base salary will depend on experience, education, geography, and other factors.
