Registered Nurse; RN - Per Diem

Job Details:
Bristol Hospice - Fall River, MA 02723. Position Type: PRN. Salary Range: $45.00 - $50.00 per hour. Shift: Day (Per Diem/PRN Monday-Friday 8am-5pm). Territory:
Greater Fall River, Taunton and below, closer to Fall River and southernmost point.

• Complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illnesses.
• Provide professional nursing care utilizing all elements of the nursing process.
• Assess and evaluate patient status by writing and initiating a plan of care; regularly re-evaluating patient and family/caregiver needs and revising the plan as necessary.
• Use health assessment data to determine nursing diagnosis and develop a care plan with established goals based on the diagnosis.
• Include patient and family in the planning process.
• Record pain/symptom management and changes/outcomes as appropriate.
• Communicate with the physician regarding patient needs, report changes in condition, and obtain/receive physician orders as required.
• Teach patient and family/caregiver self‑care techniques as appropriate.
• Work in concert with the interdisciplinary group.
• All other duties as assigned.

• At least one (1) year of acute care experience in an institutional setting.
• One (1) year of experience in home care preferred.
• Current licensure in the state.
• Understanding of the philosophy of hospice concept and the needs of the terminally ill.
• Flexibility in work hours.
• Demonstrated nursing skills per competency checklist.
• Willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs.
• Ability to travel throughout the Bristol Hospice Service Area.
