In-Home Health - Nurse Practitioner or Physician Assistant; Part Time -Bristol County, MA

Signify Health, part of CVS Health®, is seeking a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. In this role, you'll be primarily traveling – up to 100% of the time – across a defined coverage area, as defined by business needs.

This may require overnight hotel stays, car rentals and/or flights, depending on the prescribed coverage area. Holding multiple state licenses will be required, as dictated by business needs. This role is sign‑on bonus eligible.

In the traveler role, you will:

• Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes
• Be part of our efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs
• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
• Receive additional per‑completion incentive pay for all in‑home visits
• Be flexible to travel locally and within licensed states, up to 100% of the time
• Conduct virtual visits as needed
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)

As an in‑home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may present a variety of conditions. Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.

• Board certification (required)
• Able to obtain and maintain multi‑state licenses in coverage area, as required
• Able to cover all counties within Massachusetts and flex to other areas of business need
• Coverage markets include:
  Barnstable, Dukes, Nantucket, Bristol, Norfolk, Plymouth, Suffolk, Essex, Middlesex, Berkshire, Franklin, Hampden, Hampshire, and Worcester

Typical pay range: $95,738.00 – $. The actual base salary will depend on experience, education, geography and other relevant factors.
