Visiting Nurse Home & Hospice is seeking two RN Case Managers to join our busy Home Healthcare team serving Aquidneck Island and surrounding communities including Fall River and nearby towns. You should be an experienced nurse with home health background and be willing to travel to patient homes.
You will perform initial and ongoing assessments, coordinate care with physicians and the interdisciplinary team, and support patients and families with Hospice/palliative care.
