Under the direction and supervision of the Director of Advocacy & Family Support per the AFC Regulations, the LPN may:
- Complete and coordinate all applicable clinical assessments and clinical evaluations.
- Develop and review on an ongoing basis each member’s AFC plan of care.
- Provide oversight to AFC caregivers in conjunction with the care manager.
- Review the PCP Summary Form.
- Monitor the health status of all members to ensure that all needed AFC is properly delivered.
- Report changes in the health status of any member to the member’s PCP.
- Conduct on-site visits with each member at the qualified setting in accordance with 130 CMR 408.415(B)(6).
- Licensed Practical Nurse with two years’ experience with /DD populations or the elderly. Current license in the State of Massachusetts.
- Valid driver’s license and insured vehicle.
