Job OverviewWe are seeking a Supportive Housing Case Manager to enhance our current services for those who were unsheltered in our community. This program provides intensive case management to individuals who have struggled with mental health and substance use conditions. The role involves frequent transportation of clients and meeting with them in a variety of settings including public spaces, encampments, and shelters.
Working with the Housing team offers skills and contacts that will benefit future social services positions.
- Coordinate with clients, medical, and mainstream providers to ensure appropriate wrap‑around services once housed.
- Work with a team to develop programming and supports to help those in Permanent Supportive Housing remain housed.
- Assist with applications for subsidized housing and benefits to increase income.
- Collaborate with local landlords and other providers to establish housing for households in crisis.
- Provide transportation for clients to a variety of services.
- Meet with clients in the community to assist with access of services, rapport building, and advocacy.
- Work in the community and in the office, with occasional hybrid work opportunities.
- Work with collateral agencies to locate housing for clients currently experiencing homelessness.
- Complete scheduled needs assessments and individualized action plans for each client.
- Bachelor’s degree in Social Work, Sociology, Psychology, or Human Services related field OR 3‑5 years of experience working with homeless populations and/or homeless programs.
- Knowledge of available referral sources for providing services to clients, including community support and social service resources.
- Ability to drive throughout the county and occasionally transport clients.
- Ability to interact with clients, co‑workers, and volunteers with diverse backgrounds and treat each individual with respect, dignity, and compassion.
- Strong oral and written communication skills and ability to follow detailed instructions.
- Comfortable using Microsoft Office 365 tools and ability to learn variety of computer systems for recording and medical records.
- Bachelor’s degree in Social Work, Sociology, or equivalent education plus experience.
- Multilingual (able to speak and read Spanish, Portuguese, Haitian Creole, Cape Verdean Creole).
