The Hamptons Center for Rehabilitation and Nursing is seeking a Social Worker to plan and administer social service programs within the facility, collaborating closely with the Director of Social Services. The role includes discharge planning, policy development for community health services, and coordinating with community resources to support residents and families.
You will identify residents’ social needs, maintain records, and work with the treatment team to ensure appropriate support.
