Are you passionate about helping individuals build a path tolasting recovery? SSTAR’s Fall River Inpatient Program is seeking a
dedicated and motivated Case Manager to join our team!

In this rewarding role, you’ll play a vital part in supporting patients as they transition from inpatient care to continued treatment and community resources. You’ll work closely with acollaborative, compassionate team to ensure each individual has thetools and connections they need for success beyond our program.

• Provide comprehensive case management services
  focused on aftercare planning
• Connect clients with ongoing treatment, housing, and community resources
• Collaborate with a multidisciplinary team tosupport patient progress

Education: High school diploma or GED required;

Bachelor’s degree in Social Work or a related field preferred.

Strong verbal and written communication skills.

Open to working with people from diverse backgrounds, cultures and lifestyles in a non-judgmental manner.

Direct care positions in addiction programs require two years of continuous sobriety for individuals in recovery.
