Permanent Supportive Housing (PSH) Case Manager

Catholic Charities Permanent Supportive Housing (PSH) program provides supportive services to individuals and families who have experienced homelessness. The main focus of the PSH program is to help individuals and families identify barriers to housing stability and engage in goal planning that links individuals to resources and skills in order to achieve long-term housing stability. Additionally, Catholic Charities connects families to resources that are available within their respective communities to sustain long-term stable housing.

The Case Manager is responsible for working closely with the Program Coordinator and other employees to ensure a positive and safe environment for clients while ensuring the functions of the program are met. Completes initial needs assessments of clients and acts upon critical needs appropriately and immediately while developing a comprehensive, client-driven, case plan with both short-term and long-term goals. The Case Manager is also responsible for ensuring the functions of the program are met while providing services that are safe, timely, effective, efficient, equitable, and client-centered.

As a Case Manager, you'll be the primary guide for your clients — working closely with the Program Coordinator to ensure a safe, positive environment while helping clients secure resources and build long-term stability. You'll:

• Complete initial client needs assessments and develop comprehensive, client-driven case plans featuring short-term and long-term goals.
• Connect clients with essential community resources to meet their needs for employment, public assistance, education, mental health, clinical issues, and substance use support.
• Meet with clients weekly to evaluate progress, identify potential barriers, and develop actionable strategies.
• Conduct bi-weekly unit inspections, coordinate empty unit inspections with maintenance, and manage necessary screening steps like CORI forms and credit checks for new clients.
• Maintain accurate documentation, manage client case files, and handle daily tasks efficiently within the ASSIST computer program, Outlook, Microsoft Word, and Excel.
• Participate in an on-call rotation and respond to emergencies, inquiries, and client needs with professionalism and compassion.

This is a hands-on, community-facing role requiring strong organization, assessment, and communication skills to manage a dynamic caseload across scattered sites and facilities.

• BA/BS in Human Services preferred, or equivalent combination of education and experience in human services, housing, and homelessness services.
• Excellent organizational, assessment, and communication skills.
• Computer proficiency with the ability to perform daily tasks within the ASSIST computer program, Outlook, Microsoft Word, and Excel.
• A reliable, insured vehicle and valid driver's license required for travel to assigned scattered sites.
• Bi-lingual skills are a helpful plus!
