Job Overview
The Person-Centered Case Manager (PCCM) works under the supervision of the Director of Substance Use Treatment and collaborates daily with the QMHP of the Substance Use Treatment Department. The PCCM implements a strengths‑based, client‑centered model to connect clients with community resources, integrate them into recovery communities, and support increased daily functioning through the Personal Wellness/Recovery Plan (PWRP).
Responsibilities- Articulate all service provisions through timely and quality record documentation, including progress notes and coordination with third‑party providers.
- Connect clients to community resources that support a healthy and adaptive recovery lifestyle.
- Act as a proactive partner and service provider with the Reno County Recovery Response Team (RRT) to connect community members experiencing relapse or overdose to support services.
- Coordinate services and client care within the organization and with substance‑use treatment clinicians to ensure continuity of care and client‑centered quality.
- Establish healthy relationships with community resources that benefit clients.
- Maintain accurate and current outcome‑measure data for all program participants.
- Act in accordance with the organization’s mission, vision, and values.
- Abide by the Health Insurance Portability and Accountability Act (HIPAA) privacy and security regulations concerning all aspects of protected health information (PHI).
- Maintain effective communication and professional relationships with patients, representatives, team members, contractors, physicians, peers, outside agencies, and the public.

- Education:
Bachelor’s Degree in psychology, social work, or a related field. - Experience:
Completion of a Behavioral Health Services (BHS) approved PCCM in Training curriculum. - Certification: BHS certification as a Person‑Centered Case Manager (PCCM).
- License:
Valid driver’s license, insurance, and a dependable vehicle. - Preferred:
None. - Knowledge, Skills, and Abilities:
Knowledge of the substance‑use provider network and services within the state of Kansas; understanding of community resources available to people in substance‑use treatment; direct service provision experience.
May require lifting up to 25lbs occasionally and standing or walking 11‑25% of the day.
