Job Summary

Location:

Horizons, Reno County. Full Time Equivalent (FTE): 0.5.

Provides mental health support to patients, typically based upon their own experiences.

• Provide direct supportive services to individuals with mental health symptoms.
• Assist clients in generating proactive solutions to barriers regarding their recovery.
• Complete documentation in a timely and accurate manner.
• Complete trainings and attend meetings as directed.
• Participate in care coordination to collaborate with community partners, caregivers, family members, physicians, and other treatment providers regularly.
• Act in accordance with the established mission, vision, and values.
• Abide by HIPAA privacy and security regulations regarding all aspects of Protected Health Information (PHI).
• Maintain effective communication and professional working relationships with patients/clients and their representatives, team members, contractors, physicians, peers, outside agencies, and the public.

Perform other duties as assigned.

No people management responsibilities.

• High school diploma or equivalent.
• Must be at least 18 years of age and at least three years older than the youth.
• Self-identify as a present or former primary consumer of mental health services.
• Minimum of one year in stable recovery.
• Valid driver’s license, insurance, and a dependable vehicle.
• State training as identified.

• Two years’ experience working with adults in a health-related field.
• BHS Certification for Peer Support Specialists in Kansas (KDADS).

• Strong organizational skills.
• Intermediate knowledge of MS Office programs.
• Ability to develop and mentor others.
• Oral communication, interpersonal skills, flexibility, problem‑solving skills.

• Occasionally exerting up to 25 lbs, frequently up to 10 lbs.
• 11–25% of the day may involve standing or walking.
