Home Health Physical Therapy Assistant (PTA)

Eden Health -  - Responsibilities:
Performs ongoing accurate assessments of each patient and completes OASIS data sets;
Develops and implements a written plan of care for each patient;
Educates patients, their families/caregivers, and other staff in appropriate physical therapy modalities;
Develops appropriate home exercise programs tailored to the patient’s clinical status, needs, and age;
Provides, modifies, or discontinues physical therapy modalities based on ongoing assessment and physician orders;
Provides supervision and mentorship to PT assistants and completes supervisory visits as required
