• Provide skill training on target goals and objectives with measurable outcomes, as determined by medical necessity by the Qualified Mental Health Professional.
• Deliver intensive, community‑based, interactive mental health services to children and adults utilizing a strengths‑based, skill‑building philosophy.
• Participate in the development and maintenance of a client‑centered Treatment Plan along with the family and treatment team.
• Assist the youth or adult in developing a basic understanding of their symptoms and possible solutions.
• Assist the adult, youth, and family to enlist natural community supports in developing solutions.
• Coordinate mental/physical health care services and facilitate communication between the client, service providers, family, and community partners.
• Act in accordance with the established mission, vision, and values.
• Abide by HIPAA privacy and security regulations regarding all aspects of Protected Health Information (PHI).
• Maintain effective communication and professional working relationships with patients/clients, team members, contractors, physicians, peers, outside agencies, and the public.

• Perform other duties as assigned.

Does this position have people management responsibilities? No.

• Bachelor’s degree in Social Work, Psychology, or a related field – OR
• Associate’s degree plus at least 2 years of experience educating and teaching clients about self‑care and improving daily living activities.
• Combined total of 4 or more years of schooling and experience.
• CPST Case Management training, KHS training, pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.
• Completion of Adult Case Management training within 60 days of employment.
• Valid driver’s license, insurance, and dependable vehicle if the position requires driving.

• Bachelor’s degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics.
• Three to five years of professional experience in mental health services, crisis response, or a related field.

• Familiarity with community mental health services.
• Strong organizational skills.
• Intermediate proficiency with Microsoft Office.

• Occasionally exert up to 25 lbs, frequently up to 10 lbs.
• 11–25% of the day may involve standing or walking.
