Human Touch Home Health is seeking an experienced Occupational Therapist (OT) for a PRN (as-needed) home health position serving Loudoun, Fairfax, and Prince William Counties.

• Perform comprehensive in-home OT evaluations and reassessments
• Develop and implement individualized treatment plans per physician orders
• Provide skilled interventions to improve functional independence
• Educate patients and caregivers on adaptive strategies and home safety
• Complete accurate and timely OASIS documentation
• Collaborate with physicians and interdisciplinary team members

• Active Virginia Occupational Therapist license
• Minimum 1–2 years OT experience (home health preferred)
• OASIS experience preferred
• EMR experience (Well Sky/Kinnser or Homecare Homebase a plus)
• Strong documentation and communication skills
• Valid driver’s license & reliable transportation
