- Assist patients in improving activities of daily living and functional independence
- Educate patients and caregivers on adaptive techniques and home safety
- Complete accurate and timely documentation in EMR
- Communicate patient progress to supervising OT and care team
- Active Virginia Occupational Therapy Assistant (COTA) license
- Minimum 1–2 years experience (home health preferred)
- EMR experience (Well Sky/Kinnser or Homecare Homebase preferred)
- Strong documentation and communication skills
Valid driver’s license & reliable transportation
