Responsibilities- Meet with clients to assess and coordinate their care needs
- Advocate on the client's behalf with facilities, physicians and family members
- Attend appointments, schedule meetings and manage care plans for clients
- Coordinate and collaborate with trusted community partners
This is an hourly position with generous compensation, paid time off and a 401k. You will set your own schedule and hours, working full time.
Qualifications- Bachelor's degree and 3 to 5 years of relevant experience. Experience in geriatric care management, health care, medical social work or case management preferred
- Must have a computer and ability to maintain electronic records.
- Use of personal car is required. Appropriate licensure and car insurance required.

- Strong organizational and problem-solving skills
- Ability to set priorities and meet goals.
- Self-motivated and work with own initiative
- Recognizes industry trends and updates in senior care community
- Ability to evaluate and avoid inappropriate placements, duplication of services, and unnecessary hospitalizations
- Ability to provide coordinated and timely communication between family members, doctors and other professionals, and service providers.
