Inova Fairfax Hospital is seeking a dedicated Registered Nurse (RN) Patient Care Navigator – Outpatient Heart Failure to join the team. The role is full‑time day shift, Monday‑Friday 8:00 a.m. – 4:30 p.m. (must be flexible). Travel may be required to Oakville & Mark Center locations in Alexandria, as well as Woodbridge and Loudoun. Sign‑on bonus and relocation assistance eligible.

• Coordinates a patient’s plan of care which may include appointment scheduling, confirming patient attendance, transportation, assisting with referral process, and answering patient questions.
• Acts as a liaison, advocate and point of contact to patients in need of services such as screening, diagnostic treatment, clinical research participation, support services, and follow‑up/after‑care plan by helping them access the best healthcare outcomes from doctors, hospitals and other providers.
• Refers cases to the appropriate resources for assistance. Anticipates patient needs and advocates for patient rights and appropriate utilization of services.
• Conducts comprehensive assessment of the patient’s health and psychosocial needs and develops a case management plan in collaboration with the patient and their support system.
• Educates patient, family or caregiver(s), and members of the health care delivery team regarding treatment options, community resources, insurance benefits, self‑care management, etc. to facilitate timely and informed decision‑making throughout all phases of the continuum process.
• Collaborates with multidisciplinary teams to address patient questions and responses about the care process and treatment options.
• Develops/maintains reliable systems to document, track and monitor patient data.
• Performs other duties as assigned.

• Education: Bachelor’s Degree in Nursing
• Experience: 3 years of nursing experience
• Certification: Licensed in the Commonwealth of Virginia as a Registered Nurse;
  Basic Life Support from the American Heart Association

• Experience: Telemetry, ICU, CVICU, IMC – step down; heart failure experience is a plus
