Registered Nurse (RN) – Home Health

Location:

Falls Church, Virginia. Visits are performed in clients’ homes in Annandale, VA and surrounding areas.

Job type:
Ongoing, part‑time or per diem, staff employment.

• Follow a designated care plan in accordance with each patient’s needs.
• Make home visits to clients within the assigned geographic territory.
• Administer medication, provide wound care, and perform treatments and procedures as prescribed.
• Monitor clients’ conditions and report any changes to the Clinical or Client Services Manager.
• Execute, document, and follow up on doctors’ orders.
• Perform client assessments as necessary.
• Coordinate case management activities and accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet.

• Current Registered Nurse license in Virginia.
• Minimum one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home‑care experience strongly preferred, but not required.
• Graduation from an accredited nursing program, evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to assigned cases.
