As a Home Health Registered Nurse, you'll provide personalized, one-on-one care that helps patients recover from illness, manage chronic conditions, and maintain their independence while receiving care in the comfort of their own homes.

What You'll Do
- Perform comprehensive nursing assessments and skilled visits
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
- Develop and implement individualized plans of care
- Provide skilled nursing services, including medication management, wound care, disease management, and patient education
- Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
- Coordinate care with therapists, social workers, and home health aides
- Complete timely and accurate clinical documentation using Well Sky
- Educate patients and caregivers to promote safety, independence, and improved health outcomes
Qualifications
- Active Virginia Registered Nurse (RN) License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment, Critical Thinking, and Documentation Skills
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with Well Sky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
