As a Home Health Licensed Master Social Worker, you'll help patients and families navigate the emotional, social, financial, and environmental challenges that can affect recovery and quality of life.

What You'll Do- Complete comprehensive psychosocial assessments
- Identify emotional, social, environmental, and financial barriers affecting patient care
- Connect patients and families with community resources and support services
- Assist with discharge planning and long-term care planning
- Provide counseling, education, crisis intervention, and emotional support
- Advocate for patients while coordinating services with community agencies
- Collaborate with physicians and the interdisciplinary care team
- Complete timely and accurate clinical documentation using Well Sky
- Active Virginia Licensed Master Social Worker (LMSW) License Required
- Master's Degree in Social Work from an Accredited Program
- Valid Driver's License and Reliable Transportation
- Strong Assessment, Communication, and Documentation Skills
- Home Health Experience Preferred but Not Required
- Experience with Well Sky Home Health is a plus
- Passion for Delivering Compassionate, Patient-Centered Care
