Summary

The Care Coordinator works interdependently with other health care professionals to provide quality health care. Beyond the basics of the traditional medical assistant role, the Care Coordinator is responsible for guiding physicians and advanced practice providers through the clinic schedule to maximize their ability to care for their patients in a connected way. They ensure that the patient feels comfortable, confident, and satisfied with their visit.

They prime the provider to be informed and prepared upon entering each visit. They coordinate next steps and follow‑up after the visit.

Responsibilities and Duties
  • Prepares for scheduled appointments a few business days in advance to ensure that the patient has completed labs and scans that were ordered for the visit and that the provider has access to results.
  • After the patient signs in at the front desk, greets the patient and escorts them in for office visits with the physician or nurse practitioner, lab work, injections, chemotherapy and non‑chemotherapy treatments.
  • Assists patients with ambulation, transfers, and prepares the patients for examinations.
  • Obtains patient vital signs, height and weight. Discusses health history and active problem lists. Updates health maintenance screenings and medications. Documents the information by entering it into the electronic medical record.
  • Set‑up for bone marrow biopsies and any other surgical and non‑surgical procedures ordered for the patient.
  • Obtains blood specimens for analysis and processes depending on the specific test ordered.
  • Administers and documents injections as ordered.
  • Triages incoming phone calls and orders prescription refills under supervision.
  • Ensures that exam rooms and physician offices are stocked with appropriate medical supplies, equipment and paperwork.
  • All other duties as assigned.
Requirements Knowledge, Skills, and Abilities
  • Strong organizational skills and ability to multi‑task.
  • Ability to work in a team environment.
  • Excellent oral and written communication skills.
  • Strong critical thinking skills.
  • Strong computer skills.
Basic Qualifications
  • High school graduate or equivalent.
  • A certificate of graduation from an accredited program for Medical Assistants or Medical Administrative Assistants is preferred.
  • Minimum two years’ experience as a medical assistant or nursing assistant.
  • Current BLS certification is required within 3 months of hire.
  • Certified Medical Assistant preferred.
PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye‑hand coordination. Requires standing and walking for extensive periods of time.

Occasionally lifts and carries items weighing up to 40 lbs. Requires corrected vision and hearing to normal range.
